RAPID CITY AREA SCHOOLS
APPLICATION FOR FREE AND REDUCED MEALS
The Rapid City Area School District #51-4 will begin serving meals in all schools on Tuesday, September 4, 2007. Students may purchase meals at the following rates: Elementary breakfast $1.25; Elementary lunch $1.50; Secondary breakfast $1.25; Secondary lunch $1.75. Individually priced food items will be available in all Secondary schools during the lunch service.
· Please print and complete the application, then turn it into the school where your child (ren) attend.
YOU MUST COMPLETE A NEW APPLICATION FOR EACH SCHOOL YEAR.
· If your children attend more than one school in the district, please make sure that EACH school where your children attend has an application on file.
· Your child MUST PAY for his/her school lunch or BRING a lunch from home until your application is approved.
· Please call the Food Service Office at 394-4061 or the school where your child attends, if you have any questions.
· Please remember that it is the responsibility of the parent or adult household member who signs the application to completely fill out the application. If the school has to contact the household for additional information it delays the approval process, and this could cause you to pay for additional meals.
· The application must be COMPLETELY FILLED OUT before it can be approved.
· If you have received a letter from the Department of Social Services qualifying your children for free meals, you need not return the application for free and reduced price meals. RETURN the Department of Social Services letter instead.
